Referral Criteria
- The person is aged 18 or over
- The person has been diagnosed with a life-threatening illness
- The person has complex needs which require specialist supportive and palliative care (which may include physical, emotional, psycho-social, spiritual and rehabilitation needs)
- The person has a clear understanding of the reason for referral and wishes to attend
- The person is able to travel by car / wheelchair taxi
- The person is currently at hoem or has a confirmed hospital discharge date
Referrals for Day Therapy may be from you GP, Hospital Medical Team, Clinical Nurse Specialist or District Nurse.
Referral forms may be obtained by these health professionals from the Hospice and may be faxed or posted.
