Co-Ordinated Care

Co-ordinated care is ensured by :-

  • Each person having a Key Worker who co-ordinates their care.
  • G.Ps notified of outcome of initial assessment and person’s first attendance at Hospice.
  • Weekly multi-professional meetings held with Community Clinical Nurse Specialists, District Nurses and Hospital Palliative Care Team to discuss current or potential attenders.
  • Weekly multi-professional meetings held with Palliative Care Team medical staff to discuss all Day Hospice attenders.
  • Medical staff attend the Hospice daily to review attenders as required.
  • G.Ps notified regularly of any changes in treatment regimes.
  • District Nurses and Community Specialist nurses contacted regularly by telephone, as required.
  • Regular contacts with outside agencies, e.g. Crossroads, Social services, Cancer Support Groups.
