How to Join

To become a lottery member with a chance to win £1,000 each week you need to complete a Subscription Application form which can be printed from the link below.

Alternatively you can contact us on 0151 526 2968 or email catherine.osborne@woodlandshospice.org.uk with your address details and we will post you an Application form.
