Clinical Services
Referrals for all aspects of the Clinical Service can be via family doctor (GP), Clinical Nurse Specialists, District Nurse or Hospital medical team.
Day Therapy Service
The hospice is able to provide care for up to twenty people per day, aged 18 years and upwards in a purpose-built building where the atmosphere is informal, relaxed and friendly.
Once a referral for Day Therapy is received, individuals are allocated a member of the Clinical Team (a Key Worker) who co-ordinates their treatment programme. Following an initial assessment, each patient is then seen by other members of the multi-professional team and a full treatment programme is agreed.
Treatments are reviewed after the first month and, then, at three monthly intervals with a view to discharge, or continued attendance, as appropriate.
Community links are maintained with the family doctor and other community staff through telephone contact and regular meetings which are held at the Hospice.
